The Clinical Nurse Specialist shall review patients as required in all settings in the Louth (1 x 1.0 WTE- full time post)HSE South: Waterford Hospice Homecare (1 x 0.8 WTE- 80% part time post)Details of ServiceDochas Centre Specialist Palliative Care HSE Dublin North East is governed through a Directorate Model and provides Specialist Palliative Care by means of three Consultant led interdisciplinary teams to the people of Louth (including South Monaghan), Meath and Cavan/Monaghan.The service has developed in line with the NACPC Integrated Programme of Specialist Palliative Care and provides outpatient, and specialist palliative consultant services in the acute hospitals and community.The regional resource of education, research, support and advice for all palliative care activities is resourced from the Dochas Centre, Our Lady of Lourdes Hospital, Drogheda.The nursing structure comprises of a Director of Nursing, Practice Development Coordinator, Team Managers, CNS and CNM1.Waterford Hospice HomecareWaterford Hospice Movement Ltd, works with the Health Service Executive in providing a community based Specialist Palliative Care service in the Waterford City and County geographical area. The service is Consultant led.This post is based in Waterford Hospice Home Care and is part of the Waterford Specialist Palliative Care Service providing a 7 day week service, 9am 5pm.Reporting RelationshipDochas CentreOperationally to The Team Manager CNM 3 Palliative CareProfessionally to The Director of Palliative Care NursingClinically accountable to The Consultant in Palliative MedicineWaterford Hospice HomecareProfessionally responsible to and reports to:Clinical Nurse Manger 3, Home Care Team.Clinically accountable to the Consultant in Palliative MedicineWorks/liaises with: Hospital based Specialist Palliative Care Teams, General Practitioners, Public Health Nursing, Allied Health Professionals and other services which may be associated with care of patients of the servicePurpose of the Post Dochas CentreThe Clinical Nurse Specialist is a key member of the Consultant led Interdisciplinary Team providing Specialist Palliative Care in the HSE DNE Area.Waterford Hospice HomecareThe Clinical Nurse Specialist Palliative Care is a key member of the Consultant led Multi-disciplinary Team providing Specialist Palliative Care in the relevant geographical area.Principal Duties and Responsibilities Clinical FocusThe Clinical Nurse Specialist Palliative Care will: Articulate and demonstrate the concept of nursing and midwifery specialist practice within the framework of relevant legislation, the Scope of Nursing & Midwifery Practice Framework (ABS) 2000, the Code of Professional conduct (ABA 2000) Perform nursing assessments and plan and initiate care and treatment modalities within agreed interdisciplinary protocols to achieve patient/client centred outcomes: evaluates their effectiveness. Attend and contribute to weekly Multidisciplinary meetings Contribute information and support on all aspects of care to patients and their families/carers Identify health promotion priorities in the area of specialist practice Implement health promotion strategies for patient/client groups in accordance with the health agendaPatient AdvocateThe Clinical Nurse Specialist Palliative Care will: Enable patients/clients, families and communities to participate in decisions about their health needs Articulate and represent patient/client interests in collaboration with the multidisciplinary team Act as a resource contact person for patients, families/carers ensuring that instructions are regarding investigation and follow up are clearly understood. Implement changes in healthcare service in response to patient/client need and service demandEducation and TrainingThe Clinical Nurse Specialist Palliative Care will: Promote a progressive learning environment based on high quality, research based nursing care Provide mentorship, preceptorship, teaching, facilitation and professional supervisory skills for nurses and other healthcare workers. Educate patients/clients, families and communities in relation to their healthcare needs in the specialist area of practice Identify own continuing professional development needs and engage accordinglyAudit and ResearchThe Clinical Nurse Specialist Palliative Care will: Keep up to date with current research and practice Identify, critically analyse, disseminate and integrate nursing and other evidence into the area of specialist practice Initiate and participate in evaluations and audits Use the outcomes of audit to improve service provision Contribute to service planning and budgetary processed through use of audit data and specialist knowledgeConsultantThe Clinical Nurse Specialist Palliative Care will: Provide leadership in clinical practice and act as a resource and role model for specialist practice Generate and contribute to the development of clinical standards of practice and guidelines Use specialist knowledge to support and enhance generalist nursing practiceHealth & SafetyThe Clinical Nurse Specialist Palliative Care will: Contribute to the development of policies, procedures, guidelines and safe professional practice Adhere to relevant legislation, regulations and standards in the delivery of service Ensure the effective safety procedures are developed and managed to comply with statutory obligations, in conjunction with relevant staff e.g. health and safety procedures. Observe, report and take appropriate action on any matter which may be detrimental to staff and or service user care or well being may be inhibiting the efficient provision of care. Assist in observing and ensuring implementation and adherence to established policies and procedures e.g. health and safety, infection control. Ensure completion of incident/near miss forms, clinical risk reporting Adhere to department policies in relation to the care and safety of any equipment supplied for the fulfilment of duty. ManagementThe Clinical Nurse Specialist Palliative Care will: Provide an efficient, effective, and high quality service, respecting the needs of each patient/client Effectively manage time and caseload in order to meet the needs of a changing and developing service Be aware of budgetary issues and resource allocation- all the time striving for value for money Continually monitor the service to ensure it reflects current needs Implement and manage identified changes Ensure that confidentiality in relation to patient/client records is maintained Maintain accurate and up to date statistics of the service provided, including audit of patient/client contacts Represent the specialist service at local, national and international meetings as required Maintain accurate and contemporaneous records/data on all matters pertaining to the planning, management, delivery and evaluation of this service in line with HSE requirementsThe above Job Specification is not intended to be a comprehensive list of all duties involved and consequently, the post holder may be required to perform other duties as appropriate to the post which may be assigned to him/her from time to time and to contribute to the development of the post while in office.Eligibility CriteriaQualifications and/ or experienceCandidates must, on the latest date for receipt of completed application forms for the post possess: Be registered in the General Division of Register of Nurses & Midwives maintained by Bord Altranais agus Cnimhseachais na hireann (Nursing and Midwifery Board of Ireland) or be entitled to be so registered.and Have at least 5 years post registration experience of which 2 must be in the specialty or related area (Palliative Care)and Have the ability to practice safely and effectively fulfilling his/her professional responsibility within his / her scope of practiceand Must demonstrate evidence of continuing professional developmentand Must have a level 8 (or higher) post-registration National Qualifications Authority of Ireland major academic award relevant to the specialist area of Palliative Care. AgeAge restrictions shall only apply to a candidate where he/she is not classified as a new entrant (within the meaning of the Public Service Superannuation Act, 2004). A panel will be formed from this recruitment campaign and future permanent or specified purpose vacancies of full time and part time working hours will be filled from this panel. The tenure of these posts will be indicated at “expression of interest” stage.Appointment as an employee of the Health Service Executive is governed by the Health Act 2004 and the Public Service Management (Recruitment and Appointment) Act 2004.Remuneration The Salary Scale (as at 01/01/10) for the post is:Euro 47,089 47,886 48,559 49,659 50,874 52,067 53,260 54,604 55,852 (pro rata)Working WeekThe standard working week applying to the post is 39 (pro rata)HSE Circular 003-2009 “Matching Working Patterns to Service Needs (Extended Working Day / Week Arrangements); Framework for Implementation of Clause 30.4 of Towards 2016” applies. Under the terms of this circular, all new entrants and staff appointed to promotional posts from Dec 16th 2008 will be required to work agreed roster / on call arrangements as advised by their line manager. Contracted hours of work are liable to change between the hours of 8am-8pm over seven days to meet the requirements for extended day services in accordance with the terms of the Framework Agreement (Implementation of Clause 30.4 of Towards 2016).Annual LeaveThe annual leave associated with the post is to be confirmed at job offer stage.SuperannuationMembership of the HSE Employee Superannuation Scheme applies to this appointment.Existing Members who transferred to the HSE on 1st January 2005 pursuant to Section 60 of the Health Act 2004 are entitled to superannuation benefit terms under the HSE Scheme which are no less favourable to those to which they were entitled at 31st December 2004. Appointees to posts in the Mental Health Services which formerly attracted fast accrual of service should note that the terms of Section 65 of the Mental Treatment Act 1945 do not apply to New Entrant Public Servants as defined by Section 12 of the Public Service Superannuation (Miscellaneous Provisions) Act 2004.ProbationEvery appointment of a person who is not already a permanent officer of the Health Service Executive or of a Local Authority shall be subject to a probationary period of 12 months as stipulated in the Department of Health Circular No.10/71.Protection of Persons Reporting Child Abuse Act 1998As this post is one of those designated under the Protection of Persons Reporting Child Abuse Act 1998, appointment to this post appoints one as a designated officer in accordance with Section 2 of the Act. You will remain a designated officer for the duration of your appointment to your current post or for the duration of your appointment to such other post as is included in the categories specified in the Ministerial Direction.
