*,*,Click to edit Master title style,Click to edit Master text styles,Second level,Third level,Fourth level,Fifth level,ENVIRONMENTAL, HEALTH & SAFETY (EHS) TRAINING FORCONTRACTORS,Conducted by:,Safety Officer / Fire Safety Manager,Environmental, Health & Safety (EHS) Section,Advanced Micro Devices (Singapore) Pte Ltd,1,INTRODUCTION,2,Requirements,Every new contractor must attend the EHS orientation training before he is permitted to work in AMD.,In addition, every contractor must attend the EHS refresher training every year.,Failure to attend the training, the security access will be de-activated for contractors from working in AMD.,3,Objectives of Course,To train the contractors on AMD EHS procedures.,To promote EHS Awareness among the contractors.,To ensure that the contractor work practices and procedures are in compliance with Legal and AMD EHS Requirements.,To monitor contractor EHS performance.,To achieve the goal of zero accidents / incidents (zero business interruptions).,4,EHS MANAGEMENT SYSTEM,5,Background,ISO 14001:International Standard on Environmental Management System.,OHSAS 18001:Internal Standard on Occupational Health & Safety Management System.,AMD (S) was certified for both ISO 14001 and OHSAS 18001 since December 2000.,6,Elements of ISO 14001 and OHSAS 18001 Management System,Environmental, Health & Safety (EHS) Policy.,EHS Aspect / Impact / Hazard / Risk Assessment.,Objectives, Targets and Management Programs.,Legal and Other Requirements.,Resources, Structure, Roles, Responsibility and Authority.,Training, Awareness and Competence.,Communication.,Documentation and Control.,Operational Control.,Accidents, Incidents and Non-Conformance.,Emergency Preparedness and Response.,Monitoring and Measurement.,Evaluation of Compliance.,Non-Conformity, Corrective Action and Preventive Action.,Control of Records,Internal Audit,Management Review,7,Environmental, Health & Safety Policy,Commitment,Endorsement,Documentation,Awareness,Review,8,KEY LEGAL REQUIREMENTS MANDATORY UNDER SINGAPORE LAW,9,Special Provisions relating to Health, Safety and Welfare,Section 72 Safety Committees:,If,employee,headcount is 50 or more, occupier shall establish a safety committee on which both employees and management are represented.,Committee to promote co-operation in achieving and maintaining safe and healthy working conditions, carry out from time to time inspections and to inspect the scene of any accident or dangerous occurrence.,10,Factories (Safety Officers) Order,Full Time Safety Officers:,Timber Factories,employing, 100 persons.,Factories used for processing petroleum or petroleum products.,Other factories employing 500 persons.,Full-Time Safety Officer shall spend at least 40 hours per week in the factory.,Part-Time Safety Officers:,Factories used for manufacturing Garments or electronic appliances employing, 500 persons.,Other factories employing 100 persons but less than 500 persons.,Part-Time Safety Officer shall spend at least 15 hours per week in the factory.,11,Fire Safety (Specification of Premises requiring Fire Safety Managers) Notification,Building Owner shall appoint a “Fire Safety Manager:,Floor area of the building is 5,000 m and above; (Or),Buildings of 9 storeys or more (incl Basement) in height; (Or),Hospitals; (Or),Buildings having an occupant load of 1,000 persons or more.,12,WORK COMMENCEMENT,13,Roles & Responsibilities of Contractor Supervisors,Ensure that all employees and sub-contractors under their supervision comply with all provisions of this and all other applicable programs.,Ensure only trained and competent personnel are hired to work on the AMD site.,Perform daily EHS inspection of contractor work area and equipment to ensure that EHS requirements are complied with.,Take immediate action to correct any unsafe act or unsafe condition within their capacity.,Follow the requirements stipulated in the AMD,Contractor EHS Program,.,14,General Requirements,Contractors shall possess valid professional certificates/ licenses/ permits as applicable to the work that is to be performed.,Contractor supervisor shall perform daily inspection of their work area to ensure that the tasks are performed successfully and without any incident that could result in injury, illness, property damage or environmental impact.,In case of any violations, contractor supervisor shall follow up and execute on action plans to be executed to prevent the recurrence of similar violations in future.,15,Work Statement,Prior to work commencement, fill up the work statement form and submit to AMD EHS Section.,If the work schedule need to be extended beyond the period (Expected Completion Date) stated in work statement form, a new form shall be submitted to AMD EHS Section.,Not applicable to resident contractors such as janitors and cafeteria staff as they are stationed in AMD premises daily and work is pre-defined.,Declaration letter stating that workers engaged in work are physically fit to perform the tasks and letter shall state the particulars such as names of the workers and their NRIC/ work permit numbers.,16,ACCIDENT / INCIDENT INVESTIGATION INVESTIGATION AND REPORTING,17,Accident and Incident Reporting (1),Immediately report all work-related injuries, illnesses and all near misses (potential injuries or illnesses, property and/or equipment damage or hazardous conditions) to the AMD project coordinator and EHS section.,Contractor supervisor and AMD project coordinator shall arrange with EHS section for a joint investigation of any accidents or nearmisses involving their work activities.,Contractor supervisor work with AMD Project Coordinator in submitting report to EHS section in a standard format given by AMD.,18,Accident and Incident Reporting (2),The submission of injury / illness statistics shall be in standards format as required by AMD EHS Section.,All the information submitted shall be correct.,The statistics submitted to AMD shall be treated “STRICTLY CONFIDENTIAL.,19,FIRE SAFETY,20,No Smoking Rules,No smoking inside the premises.,Comply with NO SMOKING rules.,Carelessly thrown cigarette butts are common causes of fire.,21,Fire Hazardous Materials,Do,Do Not,Take precautions when transporting, storing and handling flammable liquid.,Use of approved safety containers.,Obtain Material safety Data Sheet (MSDS) of chemical and understand its hazards and precautions to be taken.,Review & approval of MSDS by EHS prior to chemical purchase (Spec 608-60126.19).,Keeping away ignition sources.,Adequate control of spillage.,Store incompatible chemicals together.,Unauthorized entry into hazardous areas.,22,Flammable Liquid Containers/ Gas Cylinders,Do,Do Not,Containers and cylinders shall be labeled indicating the contents and hazards.,Containers made of metal or other electrically conductive materials grounded when transferring liquids from one container to another to prevent the fires caused due to static electricity.,Stacking height of flammable liquid/ gas containers exceed 3.6 m for vertical stacking.,Flammable liquids left exposed in open containers.,23,General Pointers,Do,Do Not,Participate in fire drills.,Know your escape routes and assembly area.,Ensure that all equipment are properly maintained. A lack of lubrication of moving parts can lead to fires.,Passageways and exits obstructed.,Fire fighting appliances blocked.,24,EMERGENCY PREPAREDNESS,25,AMD Internal Emergency Contact,AMD Internal Emergency Telephone Number is 39111 (if using AMD Internal Telephone) or 67969111 (if using mobile).,In case of any accidents/ nearmisses/ incidents such as fire, chemical spill etc, the contractor supervisor or worker shall call the above telephone number and provide the following information:,Name of the Caller,Contractor Company Name,Location of Incident,Brief description of Incident,26,Response to Fire Emergency (1),Informant (Witness):,Person who discovers the fire shall immediately:,Raise the fire alarm by activating the nearest fire alarm “break-glass call point (in case there is no fire alarm activation in building).,Proceed to a safe location and call the AMD Internal Emergency Telephone Number (Ext 39111) and provide the information such as his/ her name & dept he/ she belongs to, location of fire and number of casualties (if any).,Attempt to extinguish any incipient fire with the available fire fighting equipment (if you are confident to do so) and without taking personal risk.,27,Response to Fire Emergency (2),Occupants:,Upon hearing the fire alarm, lock important files, cash, shutdown the machinery etc, proceed to the nearest PA system speaker and wait for further announcement.,Upon hearing announcement for evacuation or second stage continuous alarm, evacuate immediately using the nearest exits.,Follow the instructions given by the Fire Wardens/ ERT Members.,Do not panic but quickly walk down the staircase by the nearest exits and proceed to the assembly point.,Do not use lifts,.,28,Response to Fire Emergency (3),Occupants (Contd):,Give way for disabled, pregnant women etc., to ensure that there is always a clear path for these people to be safely evacuated from the building to the assembly point.,The assembly point for AMD Building Occupants is located at car park behind Blk 512 of JTC premises (near JTC Canteen).,For those working in JTC Blk 512, assembly area is located car park behind Blk 514.,Do not re-enter the building once at the assembly point unless instructed otherwise by the SCDF Personnel.,29,Evacuation DOs (1),DO:,Wait and Hear for the PA announcement for evacuation.,U,pon hearing the announcement, evacuate using the nearest emergency exits and staircases.,W,alk quickly.,E,vacuate in an orderly manner.,Us,e only MAIN GATE for evacuating from the AMD building.,Beware of vehicle movements and observe the EXTRA CAUTION when walking thru the JTC car park area.,30,Evacuation - DOs (2),DO:,7. Obey the instructions given.,Proceed and assemble at designated assembly area.,Assemble together in lot allocated for contractors.,Cooperate in headcount.,Return to the building in an orderly manner (only when you are advised to return to the building),31,Evacuation- Donts,DONOT:,Use the lifts,R,un,Use the SMALL SIDE GATE for evacuating.,Walk under the BARRIER GATE (JTC Building) as there is a potential hazard.,Leave the assembly area without notifying your immediate supervisor.,Return to the building until you are advised to do so.,32,What is In-Place Protection (IPP)?,In-place Protection (IPP) shall be executed whenever there is a hazardous substance leak in outdoor atmosphere.,Hazardous Release in outdoors- safer to stay inside the building.,Procedure to reduce infiltration of hazardous substance from entering the building.,33,Response during In-Place Protection (IPP),All Occupants:,Upon IPP Declaration:,Lock important files, cash, shut down machinery etc. Do Not make unnecessary telephone calls.,Immediately evacuate by using the nearest route and proceed to their pre-determined IPP room.,Not leave the IPP Room/ building until the IPP is terminated.,Follow the instructions given by ERT.,Assist in sealing the openings in IPP rooms.,34,IPP Rooms,In case of IPP declaration thru PA announcement, proceed to designated IPP room in respective floor as guided by their AMD Project Coordinator.,Check with your AMD Project Coordinator on location of IPP Rooms.,Contractor supervisor shall conduct headcount of his workers in IPP room and report to Security Control Room (Ext: 39781 or DID: 67969781).,35,IPP Room Logo,36,Emergency Exits and Escape Routes,Emergency exits, aisles and fire lanes shall be kept clear of obstruction at all times.,Emergency exits shall be used only in case of an emergency.,Emergency exit doors are identified by signs and installed with alarms that will sound when doors are opened.,Emergency Exit Door Alarms are linked to Security Control Room.,Staircases # 3 and #5 in AMD building are meant for emergency use only.,37,Spill Control,Contractors shall take all measures to prevent chemical or water spills/ leaks.,Contractor Supervisor shall ensure that the workers follow all the requirements and procedures.,In case of any spills (even if it is a minor one), contractors shall immediately clean up the spills.,38,HAZARD COMMUNICATION PROGRAM,39,Chemical Review,Before any chemical is brought on site:,Obtain a copy of Material Safety Data Sheet (MSDS) for the chemical from chemical supplier or manufacturer.,Fill up the chemical declaration form.,Submit completed declaration and MSDS review forms together with the copy of MSDS to AMD EHS Section for approval before commencement of work.,All the contractors shall wear the necessary personal protective equipment (PPE) and comply with all safety requirements specified in MSDS.,No chemical shall be brought on site unless prior approval is obtained from AMD EHS Section.,Not applicable to commercially available at outside departmental stores such as WD40, Paints, Thinner etc.,40,Hazard Communication (1),All Chemical containers shall be labeled to indicate the hazards of chemicals and safety measures to be taken.,Labels shall be clear and legible.,A copy of Material Safety Data Sheet (MSDS) shall be kept available at the work areas where chemicals are used (except for commercially available chemicals such as paints, thinners, WD40 etc).,All requirements stated in MSDS shall be strictly complied with.,41,Hazard Communication (2),No information should be left blank in MSDS.,Language,used in MSDS must be clear, simple and concise. Vague and ambiguous expressions shall be avoided.,All pages of MSDS shall be numbered.,Date of issue of MSDS must be indicated.,Chemical containers shall not be interchanged (eg) storing thinner in container labeled as kerosene is prohibited.,Chemical spills shall be cleaned immediately to prevent any slippery or fire hazard.,42,Hazard Communication (3),Personnel handling the chemicals shall read the MSDS and labels on chemical containers to fully understand the hazards associated with chemicals and safety measures to be taken.,Contractor supervisor shall ensure that his workers using the chemicals are aware of hazards and safety measures to be followed.,Always wear appropriate personal protective equipment like safety goggles, rubber hand gloves, facemask, apron, before handling chemicals.,Do not eat, drink, smoke or bring bold flame inside the chemical room.,No smoking or naked flame when handling chemicals.,43,WASTE MANAGEMENT,44,Guidelines (1),Contractors shall bring their own waste containers, where feasible.,If it is not possible to bring their own waste containers, the contractors shall use the AMD waste containers.,All the hazardous wastes shall be disposed into designated waste containers and segregated from general non-hazardous waste.,Incompatible wastes shall be segregated during disposal.,The contractor/ vendor can seek advise from AMD project coordinator or EHS section on above requirements.,45,Guidelines (2),All waste containers shall have lids which shall be always kept closed except when opened for disposing the wastes.,Chemical bottles disposed into waste bin shall be tightly closed before disposing to prevent residual chemicals spilling out.,Trash bags of incompatible wastes shall be segregated during transportation and disposal into waste containers inside chemical room.,While clearing the waste bins, wear PPE such as acid resistant gloves, solvent resistant gloves etc.,DONOT transport wastes using passenger lifts.,Only Goods lift shall be used for transporting hazardous wastes.,46,PERSONAL PROTECTIVE EQUIPMENT (PPE),47,Need for PPE,PPE are devices worn by persons for protection against possible injury when exposed to hazards.,PPE can only minimize the severity of injury should an accident occur or when exposed to hazards.,48,Proper Attire,All contractors shall observe proper attire while working inside AMD premises.,As a minimum requirement, all personnel shall wear covered foot wear while working inside the premises.,Wearing of slippers is STRICTLY PROHIBITED while working inside AMD premises.,49,Types of PPE,For head protection- Safety helmets,For hearing protection- Earplugs & earmuffs,For eyes protection- Goggles & safety glasses,For face protection- Face shields & hoods,For respiratory protection- Air purifying respirators & air supplied respirators,For free fall hazard protection- Safety belts & lifelines,For body protection- Clothing, aprons & gloves,For foot & leg protection- Legging & safety footwear,50,SAFETY EQUIPMENT,51,Requirements,Contractor Supervisor shall be responsible for ensuring that their workers have available and use safety equipment (i.e. portable fire extinguishers, electrical ground fault circuit interrupters etc) that are appropriate for the tasks they are to perform and consistent with specific work area requirements and/or posted signs.,If the contractors cause damage to AMD safety equipment such as fire extinguishers, call points etc, these equipment shall be repaired or replaced at contractor companys expenses.,52,ELECTRICAL SAFETY,53,DOs and DONTs (1),Do,Do Not,Switch off equipment when not in use.,Use one plug for each socket,Wiring insulated and maintained.,Check for frayed, cracked, or exposed wiring on equipment cords,Use explosion proof wiring and fixtures in the flammable liquid storage and handling areas.,All necessary safety equipment (safety glasses, insulated gloves, mats, tools and ground fault interrupters, etc.) be provided by the contractor.,Overload the,electrical circuits,Leave the heated appliances unattended.,Perform work on energized electrical circuits or equipment.,Pull or jerk the cord to unplug the equipment.,54,DOs and DONTs (2),Do,Do Not,Plug power equipment into wall receptacles with power switches in the Off position.,Unplug electrical equipment by grasping the plug and pulling.,Check the receptacle for missing or damaged parts,Replace frayed leads.,Check for defective cords clamps at locations where the power cord enters the equipment or the attachment plug.,Proper grounding of electrical equipment or appliances.,Drape power cords over hot pipes, radiators or sharp objects.,Plug equipment into defective receptacles.,Use extension cords in office areas.,55,DOs and DONTs (3),Do,Do Not,Know the location of circuit breakers that control equipment and lighting in respective areas.,Any electrical equipment causing shocks or with high leakage potential must be tagged with a Danger tag or equivalent.,Extension cords should be limited to use by maintenance personnel.,Allow temporary or permanent storage of any materials within 1 m of any electrical panel or electrical equipment,Use the consumer electrical equipment or appliance if not properly grounded.,56,HAZARDOUS ENERGY CONTROL,57,Legal Requirement,Factories (Amendment) Act 2000, Section 24A,Lock Out / Tag Out shall be established and implemented for the,inspection,cleaning,repair,or,maintenance,of any plant, machinery or equipment that if inadvertently activated or energized, is liable to cause bodily injury to any person,58,Requirements,Whenever servicing or working on an equipment or system where the unexpected release of energy or startup of equipment could cause injury, lock out/ tag out procedures shall be complied with.,Contractors shall not be permitted perform any lock out/ tag out
