Action(改善措施)及Closure(結案)五個C組成.它們分別依照以下階段進行:,1.Characterize:問題的詳細描述,包括甚麼人,甚麼事,甚麼時間,甚麼地點,怎樣的大事及數量多少,分析及對策的組員有哪些等方面;,2.Contain:對于該問題的马上圍堵,以防止其集中開來,從而造成進一步的損失;,3.Causes :該問題的根本缘由(Root causes);,4.Corrective actions:針對該問題的改善對策;,5.Closure :包括該問題對策完成的追蹤確認,結案前的再次審核,持續追蹤,確認 及 經驗教訓的吸取 .,這五個依次的階段中,我們一般用以下三種顏色來表示該問題進行到的不同階段:,綠色表示該階段已經完成了.,黃色表示該階段正在進行當中.,紅色表示該階段的工作在未來的日子中再進行
