The planned construction period is from Aug. 18, 2003 to Feb. 18, 2005. 1.2 项目描述 Description of the project通过公开招投标,CCC获得了合同号为XX的工程项目,为XXX总承包,合同范围是完成对分布在6个作业面的中间体及产品罐区的基础、罐体及其附件的EPC服务,总罐容为28.5万立方米,总罐数为63个。CCC is rewarded a project which contract number XX by public invitation to bid, and act as a EPC contractor of intermediate and product tanks of XXX. The scope of contract is to complete the EPC services of foundation, tank body and assessories of intermediate and product tanks that distributed in six working areas. It covers: n 在项目之初制定、签署、发布和强调项目HSE政策;Draw up, sign, issue and stress HSE policy at the beginning of the project. n 从个人和公司的角度身体力行起到领导的表率作用,完全兑现在健康、安全和环境各个方面的承诺;Personally act as a pioneer from individual and the company s aspect, and completely perform the commitment for health, safety and environment. n 通过实施建立在行为基础上的HSE计划、HSE领导会议和HSE领导层行为审查来统一HSE责任;Unify HSE responsibility by carrying out HSE plan based on behavior, by carrying out examination on HSE leader conference and the behavior of HSE management level. n 通过范例和指示,批准并确保把HSE考虑因素纳入所有项目决定的过程;Approve and assure to bring HSE factors into the decision process of all project by following examples and instruction. n 从设计阶段到施工完成,在每一次工作活动中把HSE表现作为一个评价内容来看待;View HSE as an appraisal content each time performing the work from engineering to the completion of construction. n 确保程序正常运行,积极地提前鉴别和消除任何潜在的安全或环境不良方法习惯;Make sure the smooth operation of whole program, actively distinguish and eliminate any potentially improper habit in safety and environment in advance. n 确保各级管理层的决定与规定的政策一致而且战略目标行之有效; Assure that the decision of all management level is consistent with the stipulation of policy and assure that our strategy is effective. n 给HSE计划分配适当的资金和资源使之实现既定的各种目标; Allocate appropriate capital and resource to HSE plan so as to fulfill various established target. n 在日常工作活动中以身作则,树立起一个HSE卓越表现的榜样;Practice what we preached in daily work and set an excellent HSE example. n 在整个项目中对HSE卓越表现进行积极肯定;Actively confirm HSE excellent behavior during performing the whole project. n 亲自进行HSE审查并通过的交流向所有职工提供审查结果;Personally carry HSE examination and provide examination results to all staff through communication. n 在所有管理层和职工中慢慢培养一种信念,在HSE管理系统中,每一个工作者都有机会和义务为实现战略目标做出他/她的贡献。Gradually train all management staff and employee the belief that every one has the opportunity and obligation to make his/her contribution to the realization of our strategy during implementing HSE management system. 3.2 现场指导思想 Site PhilosophyCCC致力于所有事故均可避免这一理念,因此项目部承诺要通过不断的改进方法达到并保持“零事故”的目标。CCC try to make all accidents avoid this philosophy, so project team promises to achieve and keep the “zero accident” goals by continuous improvement.4.0 政策和目标 POLICY AND OBJECTIVES4.1 政策 Policy 本项目中HSE政策是对下列内容的一种承诺:The HSE policy for the project is a commitment to:n 不伤害人员; Pursue the goal of no harm to people; n 保护环境; Protect the environment; n 建设工厂和提供服务时有效地使用材料和能源; Use material and energy efficiently to construct the plant and provide services; n 按照这些目标提供能源、产品和服务; Provide energy resources, products, and services consistent with these aims; n 以适当的形式向公众发布CCC项目部的HSE表现; Report on HSE performance of CCC project team in suitable form for public release; n 在我们行业中,带头提高HSE业绩;Play a leading role in promoting best practice in our industry;n 如同管理任何重大商业活动一样管理HSE事务; Manage HSE matters as any other critical business activity; n 用一个完整的管理体系建立各级HSE管理责任;并 Use an integrated management system to make HSE a responsibility of line management; and n 创立一种文化氛围使所有项目职工共同承担这个义务。Promote a culture in which all project employees share this commitment. 4.2 目标Target 本项目的HSE目标是将安全、健康和环境管理落实到工程项目的施工作业活动中,遵守PMC的政策目标,追求最大限度的不发生事故、不损害人身健康、不破坏环境。The target of this project is to finalize our safety, health and environment management in construction project, to strictly follow PMCs policy target to maximally avoid accidents, body injury and contamination of environment.n 宣传和积极追求零事故目标;Publicize and actively pursue zero accident target;n 努力消除一切职业伤害和疾病;Do our best to eliminate all occupational hazards and employment diseases;n 在设计、计划、培训和工程施工过程中按常量来提高HSE目标;Improve HSE target according to constant quantity during engineering, plan, training and construction. n 提高员工的HSE意识,并使之自觉加入到HSE计划的执行过程中来;Improve employees HSE ideas and let them actively implement HSE plan. n 增强员工在其日常工作活动中始终使用安全的方法与工作习惯;Train employees to use safety methods and habit in their daily works all the time. n 优先选用不断改进的方法作为零事故起步的基础,努力做到“五无”,即无死亡、无重伤、无中毒、无火灾、无坍塌。Give priority to those continuous improvement method as the basis of zero accident and try best to fulfill target of “five no”, namely no death, no serious injuries, no poisoning cases, no fire disasters and no collapses. 5.0 HSE组织机构 HSE ORGANIZATION HSE队伍组织机构设置如下图5.1,通过它CCC项目部将执行项目的HSE管理制度,它负责整个项目范围的所有HSE计划制定及管理。HSE是一种贯穿所有部门的综合性责任,HSE队伍负责制定通过线式管理来执行的原则,并逐级下传,HSE队伍还负责监控执行的情况。Organization chart of HSE team is shown as following figures 5.1, through which CCC project team will implement HSE management system, HSE team will be responsible for all HSE plan and management of the whole project. HSE is a kind of comprehensive responsibility that runs through all departments. HSE team will be responsible for developing the principle of implementation by line style management and pass down level by level. HSE team also is responsible for supervising implementaion.图5.1 HSE组织架构图 Figures5.1 Organization Chart of HSE图5.1 HSE组织架构图 Figures5.1 Organization Chart of HSE6.0 责任、资源、能力及标准 RESPONSIBILITIES, RESOURCES, COMPETENCY AND STANDARDS6.1 人员职责 Responsibilities6.1.1 项目经理 Project Manager n 是本工程项目HSE工作第一责任人,对本工程项目HSE工作全面负责;Who is the principal of HSE and be fully responsible for the HSE works of the project.;n 负责组织本项目部HSE管理体系实施进行;Be responsible for organizing and performing HSE project management system; n 主持召开本项目部HSE例会,及时解决HSE工作中的问题,并定期参加PMC组织召开的HSE会议;Chair HSE regular meeting of the project, solve the problems arise from the HSE works in time and periodically takes part in the HSE meeting held by PMC;n 积极参与书面形式的岗前计划活动,参与PMC实施的定期的书面形式的安全、健康与环境评估;Actively take part in pre-construction written activities, and take part in periodical written evaluation on safety, health and environment held by PMCn 定期对项目部组织HSE监督检查,及时进行隐患治理,不断改善施工作业条件;Periodically organize HSE supervising and checking to project group, timely find and solve the hidden troubles and continuously improve construction conditions. n 支持项目安全员及施工管理人员行驶安全监督、检查和督促、检查工作;Supports the project safety supervisor and construction superintendent in the inspection and supervision of safe driving, and in the inspection and supervision of workn 组织项目部事故的调查、分析和处理并及时上报。Organize to investigate, analyze, handle the accident of project group and report it to supervisor in time.6.1.2 项目施工经理 Construction Managern 根据项目安全保证计划组织有关管理人员制定针对性的安全技术措施,并经常督促检查;Organizes the relevant superintendents to formulate the pertinent technical measures for safety as specified in the project safety guarantee plan; and conducts the frequent inspection and supervision;n 协调HSE运行中的重大问题,组织召开安全生产工作会议;Coordinates the settlement of significant problems in the operation of HSE; and organizes and calls the safe production work meeting;n 定期组织管理人员进行安全操作规程和安全规章制度的学习;Regularly organizes superintendents to study safe operation rules and safety system regulations;n 实施现场安全管理标准化,确保操作现场工作环境不影响施工安全;Standardizes the safety management in the work site so as to prevent the work environment in the operation site from affecting the construction safety;n 组织安全设施的验收,协助上级部门对工程项目的安全检查和督促;Organizes the inspection-acceptance of the safety facilities; and assists the superior organizations in the inspection and supervision of the project safety;6.1.3 项目HSE经理 HSE Manager n 代表项目经理全面负责项目的HSE体系的实施工作,对项目HSE工作负直接的领导责任;Be entirely responsible for implementation of HSE system of the project on behalf of Project Manager and directly take the leading responsibility for project HSE work. n 组织制定项目HSE的目标与计划,并组织实施;Organize to establish the target and plan of HSE and to implement;n 组织协调项目HSE管理体系的实施,纠正不符合HSE管理体系文件规定的行为; Organize and coordinate the implementation of HSE management system, and correct the behaviors that are not compliance with the stipulations of HSE management system documents. n 定期参加PMC组织的书面形式的安全、健康和环境评估,积极参与书面形式的岗前计划活动;Periodically take part in written evaluation on safety, health and environment held by PMC and actively take part in pre-construction written activities,n 定期组织项目HSE检查,落实事故隐患的整改。Periodically organize HSE check and perform the corrective measures to hidden troubles 6.1.4 项目HSE管理组 HSE Management Group n 贯彻执行国家、地方政府、PMC和公司的有关HSE方针、政策、法律、法规、标准和规定;Carry out the relevant HSE guidelines, policies, laws, rules, standards and stipulations established by the State, local government, PMC and the company. n 审定项目部的HSE工作计划,监督检查HSE管理工作的实施情况;Examine and approve the HSE working plan of project group, check and supervise the implementation of HSE management works. n 定期召开会议听取项目部的HSE工作情况报告,研究解决HSE工作中遇到的问题。Report the HSE works of project group in the meeting held periodically, and solve the problems arise from works.6.1.5 项目主管人员 Project Management Staffn 支持、促进和参与项目的零事故管理小组;Support, promote and participate zero accidents managing group of the project;n 严格监督实施本工种的安全操作技术规范;Conducts the strict supervision of the implementation of the safe operation technical rules as required in the work of this type;n 有权监督不符合安全操作的施工任务,除及时制止外,有责任向项目HSE经理汇报;Is authorized to conduct the supervision of the construction task in violation of the safe operation regulations; and obligates to report to the project HSE Manager apart from stopping the irregularities;n 认真执行对施工人员的分部、分项工程有针对性的安全技术交底;Seriously communicates the safety technology with regard to the partial work and subproject to the construction employee;n 发生工伤事故,应立即采取措施,并保护现场,迅速报告;Immediately Takes measures to protect the site and submits the report in time after an injury accident occurs;n 对已发生的事故隐患落实整改,并向项目HSE经理反馈整改情况;Executes the correction of the potential accident which appears; and submits a report with regard to the correction to the project HSE Manager;n 以书面形式记录所采取的安全、健康和环境书面的所有行动;Record all the actions taken for safety, health and environment in a written form;n 对安全作业给予书面支持和认可,积极参与书面形式的岗前计划活动,参与项目基于作业基础的安全程序。Give written support and confirmation to safety construction, actively participate pre-construction written planning activities and implements safety procedures that the project is based on construction basis.6.1.6 HSE监督员 HSE Surveillantn 参与制定有关HSE目标计划、技术措施,监督检查执行情况;Participates in developing HSE goals and technical measures, supervise and inspect execuation n 负责组织员工进入现场前的HSE教育、岗位HSE教育;Responsible for organizing HSE education for employee before entering into jobsite and on jobn 监督、检查、参加、指导班组每周HSE活动及班前讲话;Supervise, inspect, participate and instruct groups on weekly activities and give talksn 深入现场巡回检查,对各种直接作业环节进行HSE监督,及时纠正违章和失职行为,督促检查隐患整改,遇有紧急情况有权令其停止作业;Go to site for a thorough inspection, supervise each direct work steps on HSE aspect, correct the violations and breaches of duty in time, and urge to check the hidden troubles and improve, have the right to stop operations when emergency.n 及时向领导反映问题,提出建议,为领导HSE决策提出建设性意见;Report problems to leaders in time and propose the suggestions, offer useful ideas on HSE decision for leaders.n 参加项目部HSE检查、HSE管理评审及事故的调查处理;Participate in HSE check, HSE management review and accident investigation and handling.n 负责有关HSE管理记录、台帐、资料的整理、归档和上报工作。Responsible for HSE management records and documents keeping, filing and reporting.6.1.7 班组长 Group leadern 保证上级各项HSE标准、制度在本班组的贯彻执行,对本班组的HSE工作全面负责;Ensure all HSE standards and regulations from superiors are implemented and be responsible for overall HSE works of this group. n 负责组织员工的岗位HSE教育和班组HSE活动,学习有关HSE操作规程;Responsible for organizing HSE education on job and HSE activities of group for employee, and studying related HSE operation instruction.n 认真检查班组各岗位作业中的HSE执行情况,发现违章和隐患及时处理;Check carefully HSE performance of each work, deal with violations and hidden troubles in time when finding them.n 做好有关HSE防护措施、防护用品的检查维护工作,使其处于完好状态;Check and maintain related HSE protective measures and articles, and ensure them in a good conditions.n 发生事故抢救伤者,保护现场,并立即报告,对事故处理予以积极协助。Salve injured persons when accident, protect site and make an immediate report, and give an active assistance at accident handling.6.1.8 员工 Employeen 严格遵守自己的安全承诺,严格遵守各项HSE管理制度及本岗位操作规程,不违章作业,对本岗位的HSE工作负直接责任;Comply with the safety promises and all HSE management regulations and operation instructions of the position strictly, will work without any violation, have the direct responsibility to the HSE works of this position.n 积极参加班组每周及每日班前HSE活动,自觉接受HSE教育培训,增强自身HSE意识及防护能力;Participate in weekly and daily HSE activities actively, take HSE training and increase HSE awareness and improve protection ability.n 上岗按规定着装,妥善保管、正确使用各种劳动防护器具和消防器材;Dress according to related regulations before work, keep and use kinds of work protective equipment and fire fight equipment in a correct way.n 有权拒绝接受不符合HSE管理规定的作业任务,有权拒绝违章作业的指令,对他人的违章作业有权加以劝阻和制止;Have the right to refuse to take the works and tasks which are not in compliance with HSE regulations, have the right to refuse violating instructions, and have the right to discourage and stop the violating operations of others.n 正确分析、判断和处理各种事故苗头,把事故消灭在萌芽状态;Analyze, judge and handle correctly kinds of accident at the early beginning in order to solve the problems at the early beginging.n 发生事故要果断正确处理,及时向上级报告,并保护现场。Handle the accidents correctly and determinedly, report to superior in time and protect site.6.2 标准 Standards具体的中华人民共和国健康和安全标准参考附件1。 Specific PRC Health and Safety standards are referenced in Attachment 1. 中华人民共和国环境标准在HSE环境监控程序(附件2)中叙述。The PRC Environmental standards are described in the Construction Environmental Control Plan (Attachment 2).7.0 执行和通讯 Implementation and Communication 7.1 语言 Language 所有的会议和报告都将以普通话和英文两种语言方式打出或呈现,必要时地方话可用来保证完全理解。其它需要注意的重要考虑因素包括:All meetings and reports will be typed or shown both in PuTongHua and English. However, dialect, if necessary, can be used for being understood fully. Other important factors need to be taken into consideration are: n 相对口头交流而言,重点应该以书面形式;Compared to oral communication, key problems should be shown in written documents;n 在书面函件上中文应该有优越的字号并置于优越的位置;Chinese should be used with preferential size and demonstrated at prevailing position in a written document. n 公司和团体之间建立通讯界面;Communication interface should be established between the company and groups. n 公司和团体建立各种交流形式的详细程序(如:常规通讯、事件汇报、职权机构的通讯、紧急事故报告和变更的管理)。Detailed procedures of various communication between the company and groups are established (such as
